Thank your for your interest in the Mentor of the Year Award. Now that we have your information, we can contact you if we have any questions. However your job is not done just yet. Please provide the following documents:
Document 1 | A list of sites mentored from January 1st to December 31st of the previous calendar year. (Document 1 can be- PDF,DOC,DOCX)
Document 2 | Three letters of recommendation – one from a site you mentored, a supervisor of yours, and a co-worker. (Document 2 can be- PDF,DOC,DOCX)
Document 3 | A picture of you or the person you are nominating. (Document 3 can be- JPG,TIF)
Document 4 | You will also be asked for a brief description on how you meet the below criteria (Document 4 can be- PDF, DOC, DOCX):
- Scope of activities such as – Number of assisted VPP onsite evaluations, number of potential VPP sites mentored, number of OSHA training co-instructed (i.e. SGE course, OSHA Ed Center courses).

The winners will be recognized at the Regional Conference and at the National Conference. The photo that you supply will potentially be placed in the conference related publications.
