It is 2025, but we are using your recorded 2024 data and comparing it to 2023 or 2022. The reason the comparison years (2023/2022) are used is due to when this award application comes online and when the prior year rates are published. This always allows us to have proper comparative data.
If you submit multiple times for the same site, we will honor the very last entry for that site.
Questions
Q. I don’t quite understand the rules, can someone help?
A. Your questions about the rules can be directed towards your Dallas (formerly Region VI) OSHA Regional Area Representative, Noel Buitrago.
Q. The field is not letting my insert my numbers. Why not?
A. The NAICS code for the year you selected for comparison and your injury and illness rates for 2024 are required for you to input, but the other fields are auto populated based on BLS data for the comparison year you selected and an auto calculation is made and an award level assigned.
Q. I inserted my numbers but the form will not let me submit.
A. If you do not receive a qualifying score in the Level of Recognition field, this form will NOT let you submit the entry. Filling in the NAICS code for your industry WILL automatically fill in your industry information including the average industry rates for the year you are comparing against.
Q. The page does not load correctly and nothing is auto calculating for me. What do I do?
A. This form REQUIRES THE USE OF JAVASCRIPT. If javascript is being blocked by your worksite, use a different non-work network or a different computer such as your home computer (It even works with most phones!)
