Leading in High-Risk Environments:  Get This Right
In these lean times, we’re all expected to be effective managers and inspirational leaders.  These similar, yet subtly different skill sets don’t always readily coexist, and day to day practicalities seem to demand we wear our management hat most of the time.  Alarmingly, data from the US BLS suggests SIFs are on the rise.  Paradoxically, it is also the safest time ever in US industry with respect to slips, trips, and falls.  How is it possible to be more focused on safety and accident prevention than ever before and yet see the very accidents we fear the most become more common?!?  Discoveries in cognitive science can both help explain these disturbing trends in the data and suggest a way forward. Effective On-the-Job Training
During this session, we will cover the three (3) elements of effective OJT, observation, performance with coaching and evaluation. During the observation element, the trainer will demonstrate correct performance of all performance requirements for each job position. The trainee does not perform any work during this element. They concentrate of observing correct task performance. During the performance with coaching element, the trainee performs each performance requirement while the trainer is constantly observing and coaching the trainee through correct performance of the tasks. This element should continue until the trainee feels comfortable in performing each task independently. During the evaluation element, the training process is stopped.Evaluation is not training.It is evaluating whether the trainee is ready to perform job tasks independently.100% correct performance is required.You either can operate a dozer or you can not.You can not operate it 90% correctly.

The matrix includes involving leadership, identifying the organizational structure, transferring, and assessing skills and knowledge/experience, demonstrating proficiency, and overcoming obstacles. The competency matrix is presented in a phased-model approach, with each step up through the model building on the preceding phase. To be sustainably integrated into any organization, the matrix should be accompanied by a barrier analysis. If not properly addressed, barriers can derail the adoption of the process safety competency matrix.

Safely Improving Construction Productivity
Since 2019, the number one concern across industry was the availability of skilled workers and its implications on productivity, profitability and of course, safety. At a leading industry conference 300 operational excellence and planning leaders committed to addressing solutions about the dwindling number of workers to support the petrochemical industry. Pandemic notwithstanding, industry today is facing unprecedented challenges getting work done. Workforce and safety professionals were tasked with researching other plants around the world to identify solutions of how their companies and the newer plants were safely improving productivity with fewer people. The presentation, Safely Improving Construction Productivity, shows several new technologies identified and are being tested and utilized by Gulf Coast refineries and chemical plants today. Case studies from area plants show the results and financial impact values of systems such as connected worker systems, artificial intelligence for compliance and dropped objects elimination, proximity badges and much more. Human Performance can help us eliminate fatalities and other life altering injuries by enabling us to see beyond ‘human error’.   Human Performance (HP) is a risk-based philosophy which recognizes that error is part of the human condition and that organizational, task and individual factors greatly influence employee actions and the likelihood of success. HP sees human error as a potential symptom of trouble deeper inside a system. Trenching and Excavation Tragedy-Personal Reflections
Excavation and trenching are some of the most dangerous construction activities. About 35 fatalities occur each year in the United States on average, and about two-thirds of those fatalities occur in trenches that are less than 10 feet deep. In a recent 2022 event in Texas, two workers were killed when an unprotected trench more than 20 feet deep collapsed. Life-saving trench shields sat unused by the site.  In a more personal experience, I lost my brother to a trenching accident in 1987. The ramifications and impact of the accident have affected my family tremendously since that day. Protection and safety are paramount, and the consequences of not implementing safeguards, as I have witnessed firsthand, are long-lasting and scarring.

Annual Evaluation…..More than just a review!
The sites annual evaluation can be more than just a review of the program. The McKee Refinery utilizes this review to build working relationships while participating in a valued in depth look into our site implementation of VPP.Hourly and salaried employees from across the site team up and thoroughly analyze one to two sections of the evaluation together. The different aspects of their daily duties and work experiences bring valuable knowledge to the table and provide different points of view of the topic at hand. Over the last 3 years the site has used over 80 employees to perform this duty and provide valuable actionable data that has driven continued improvement.The value of this method of performing our annual review expands beyond participation. It gives our employees throughout the refinery ownership in the program while educating and building stronger work place relationships.

We will also review our 2023 EHS Week and have onsite hands-on demonstrations and prize giveaways. Below is part of an article written about our 2023 EHS Week. GF Central Plastics EHS Week Sets the Bar for Safety Excellence! In a week packed with empowerment and knowledge, GF Central Plastics EHS Week took center stage on November 27. Over 550 employees went through their annual safety training during the week. 14 dynamic classes, spanning crucial topics from Stop work authority, HAZCOM, fire safety, PPE, hearing, hand safety, emergency preparedness and more captivated the attention of its dedicated workforce. Each class lasting around three hours, had 40 to 50 engaged employees per session for a total safety immersion. Following the insightful classroom sessions, the employees teamed up in groups of 4 to 6 and completed 10 interactive hands in demo booths. The booths were both fun and engaging to the employees. The booths covered topics like working impaired, mental health, fire safety, hand safety, and CPR/Heimlich. After the training, employees were awarded prizes from each booth – safety shirts, hats, cups, fire blankets were given away to reinforce the commitment to safety throughout the facility. GF Central Plastics EHS Week was more than an event; it was a celebration of knowledge, camaraderie, and a united commitment to fostering a safer workplace

The   phrase “my brother’s keeper” is a reference to the Biblical story of Cain and   Abel from the book of Genesis. It is generally understood to mean being   responsible for the welfare of a brother or other sibling or, by extension,   for other human beings in general. Cain, claimed not to have this   responsibility.  We ask our employees to be “my brother’s keeper” what does that mean? We know the meaning of brother and keeper but how do we become one? Using Emotion with VR to Increase the Chance for Transfer of Learning
In the summer of 2023, the Health and Safety Council (HASC) conducted a Virtual Reality (VR) investigation to determine how effective VR could be as a training tool when combined with emotionally resonant videos for contractor learners who come to HASC for safety or skills training. This investigation was developed as a part of HASC’s VP of Learning and Innovation’s dissertation for PhD in Adult Learning and Leadership.  Studies have shown that learners who were taught in VR instead of just via traditional video or lecture-based training had an improvement in recall and retention of 28% and that both learners’ understanding and memory retrieval were greater using VR compared to traditional learning styles. By applying an emotional component, it is HASC’s hope that the transfer of learning or memory retrieval by the learner is improved.  HASC’s investigation included data from over 180 different learners who replied via text three days after the completion of the training. These participants took one of three different trainings: (a) non-VR training, (b) VR training, and (c) VR training with an emotionally resonant video. The results of the experiment will be reviewed by the audience and the participants of the presentation will have the opportunity to provide information similar to the research methodology via their own text quiz.

Safety, Psychology and Metrics:  Oh My!
For decades we’ve tirelessly worked to improve safety systems. There’s no doubt that today’s job sites are much better places to work than those of yesteryear. It’s a direct result of our significant efforts to improve safety engineering, best practices, PPE etc. We dedicate more and more effort and resources to reducing accidents and injuries in the workplace, because this is simply the right thing to do. Even so, we seem to have hit a wall. Recently, we’ve seen an increase in the number of pedestrian fatalities in the US, traffic fatalities in general have risen by 18.4% as compared to the same period last year (with only a 13% increase in miles traveled), and worst of all we’ve seen a recent increase in fatal accidents according to data from the USBLS. Clearly, we must do something differently.  Is it possible to understand why humans interact with risk in the ways that we do?Can we quantify or measure psychological safety?

Human Performance
Why is understanding Human Performance and the importance it plays on improving safety, when we have safety Regulations & Standards, enhancements to facilities & equipment, management systems, etc.? There have been tremendous strides in safety performance in industry and we have seen the number of people getting hurt decrease through the application of these different systems and techniques, but our industry has we have not seen the same progress toward eliminating life altering events and fatalities. Human Performance can help us eliminate fatalities and other life altering injuries by enabling us to see beyond ‘human error’.   Human Performance (HP) is a risk-based philosophy which recognizes that error is part of the human condition and that organizational, task and individual factors greatly influence employee actions and the likelihood of success. HP sees human error as a potential symptom of trouble deeper inside a system. HP is about understanding how people interact with plant, processes and each other – as part of a system – to help manage risk and keep people safe.

There is still a lot of misleading, inaccurate, and incorrect information regarding Selecting the proper FR Clothing – OSHA 1910.132 makes it clear that training on PPE is required. How do you train on FR clothing? What is your responsibility for care and maintenance of your FR clothing? We will cover what guidance is provided by the standards and review best practices in selecting, using, and caring for your FR clothing to assist in your organization being compliant. Per OSHA “Flame-Resistant (FR) Clothing is required under 29 CFR 1910.132 and is also mentioned in other sections of the OSHA standards. Flame Resistant clothing is used in the petroleum industry, electrical industry, O&G Exploration, and other types of industries where there is a danger of a worker being injured by the various thermal hazards.  The proper selection of PPE is very important. In this presentation, you will see that though the thermal hazards may be different for different industries the basics for selection, use, care, and maintenance of FR Clothing share some similarities. The sites annual evaluation can be more than just a review of the program. The McKee Refinery utilizes this review to build working relationships while participating in a valued in depth look into our site implementation of VPP.Hourly and salaried employees from across the site team up and thoroughly analyze one to two sections of the evaluation together. The different aspects of their daily duties and work experiences bring valuable knowledge to the table and provide different points of view of the topic at hand. Over the last 3 years the site has used over 80 employees to perform this duty and provide valuable actionable data that has driven continued improvement.The value of this method of performing our annual review expands beyond participation. It gives our employees throughout the refinery ownership in the program while educating and building stronger work place relationships.

NFPA 70E (2024) compliance/electrical safety continues to evolve, so learning when, why and how to re-visit the arc flash risk assessment and PPE program selection has become even more critical. The key NFPA 70E (2024) takeaway regarding arc flash risk assessment/PPE selection is this edition emphasizes the importance of assessing the condition of equipment maintenance – see new Informative Annex S! Properly maintaining equipment is critical in protecting your organization’s electrical workers! Improperly installed and/or maintained equipment/overcurrent devices will 1) increase the likelihood of an arc flash incident occurrence and 2) likely produce incident energies higher than predicted in original risk assessment as poorly maintained overcurrent devices will produce longer clearing times that give rise to higher incident energies, which can result in arc flash clothing/PPE being worn that under protects unsuspecting electrical workers!    What is the condition of maintenance of equipment at your facility? The answer impacts the risk assessment as well as the PPE program selection. What is the condition of your organization’s PPE-FR/AR clothing program?
